Assistance With Life Stage Transition — Compassionate Community Care
All Services Compassionate Community Care Service

Assistance with life stage transition.

Planning and coordination support through the bigger changes — moving out of the family home, changing accommodation, leaving school or work, or a significant change in support needs.

Funded Under

Capacity building — support coordination and life transition.

Covers

Moving, changing supports, education and employment transitions.

Approach

Planning ahead rather than reacting after the change.

Related

Hospital and custody transitions have their own dedicated service.

Service overview

The changes that are planned still need planning.

A move out of the family home, a change in accommodation, the end of school, a significant shift in support needs — these are usually visible well in advance and still tend to arrive as emergencies because nobody owned the preparation.

This support covers that preparation: working out what the change requires, getting the supports and services lined up, and being there through the adjustment period afterwards rather than disappearing once the box is ticked.
