Referral criteria

Who we support, and what we need from you.

Two minutes here saves a week of back-and-forth later. Here's how to know if we're the right fit, and what to include so we can respond quickly.

Who we can support

  • NDIS participants with an active plan, or a plan in progress, living in Cairns or Far North Queensland.
  • Psychosocial and mental health needs — a core strength of our nurse-led team.
  • High intensity and complex care needs, including community nursing and behaviour support.
  • Participants with a forensic background — we're one of the few local providers with a dedicated forensic support service.
  • First Nations participants seeking culturally safe support, backed by our Aboriginal Liaison Officer.
  • SIL and accommodation referrals — ask us about current and upcoming vacancies.

What to include in your referral

  • Participant details — name, contact and preferred communication.
  • NDIS number and plan dates, plus how the plan is managed.
  • Funded support categories relevant to this referral.
  • Supports you're seeking and roughly how many hours or what schedule.
  • Risks and safeguards — anything our team should know to support the participant safely.
  • Participant consent to share their information with us.
Missing something? Send what you have — we'd rather start the conversation than have a participant wait for a perfect form.

    Coordinator FAQ

    Quick answers for busy coordinators.

How quickly do you respond to referrals?+
Our intake team reviews every referral as it arrives and comes back to you promptly with either next steps or honest questions. If it's urgent, call 0435 330 666 and tell us — we'll prioritise it.
Do you take participants with complex or forensic backgrounds?+
Yes. Complex support is a strength of ours, not an exception. Our team is led by a Registered Psychiatric Nurse and includes a dedicated forensic support service, behaviour support and high intensity supports. Share the full picture in your referral and we'll give you a considered answer.
What updates will I get after supports start?+
You'll receive progress updates and notes to support your own reporting, and we'll flag changes or concerns early rather than saving them for plan review time. Tell us in the referral how often you'd like to hear from us.
    Can you support agency-managed participants?+
Yes — we're a registered NDIS provider (registration 4050097980), so we can support agency-managed, plan-managed and self-managed participants.
What if I'm not sure you're the right fit?+
Call us and talk it through. If we're not the right provider, we'll say so quickly and honestly, so your participant isn't left waiting. No hard feelings — a good referral network helps everyone.
Do you have SIL or accommodation vacancies?+
Vacancies change regularly. Send an enquiry through the referral form or call us for the current picture across our SIL, SDA, STA and MTA options.

Got a participant in mind right now?

Send the referral while it's front of mind. It takes about five minutes, and our intake team will take it from there.
