Community Access & Core Supports — Compassionate Community Care
All Services Compassionate Community Care Service

Community access and core supports across Cairns.

The everyday supports that make a week work: help at home with daily living, and support to get out into the community and stay part of it.

Funded Under

Core supports — daily living and social and community participation.

Delivered

In the participant’s home and out in the community.

Transport

Fleet vehicles and vans available.

Flexibility

Hours shaped around the participant’s week, not a fixed package.

Service overview

The support most participants use most of the time.

Core supports are the least glamorous part of the NDIS and the part that does the most work. Getting up, getting fed, getting the washing done, getting to the appointment, getting out of the house for something that is not an appointment. It is the difference between a plan that is being used and a plan that is sitting there.

We deliver both halves — the assistance at home and the community participation — because splitting them across providers usually means the community half quietly stops happening.

Who This Service Is For

Participants with Core supports funding who need practical assistance at home, support to participate in the community, or both.

Referral steps

From referral to first shift, step by step.

No mystery, no black hole. Here is exactly what happens after you get in touch.

  1. 1

    Referral

A participant, family member, Support Coordinator or hospital team sends through the enquiry.

    Form, email or phone

  2. 2

    Immediate screening

We check the request against our current capability, capacity and staffing.

    Same day where we can

  3. 3

    Funding & support needs review

We look at the plan, the support hours and what the participant actually needs day to day.

    Plan and hours checked

  4. 4

    Risk & compatibility assessment

For shared arrangements this step is never skipped, however quickly a placement is needed.

    Never skipped

  5. 5

    Options & stakeholder discussion

We present the accommodation or support options and talk them through with everyone involved.

    Everyone at the table

  6. 6

    Consent & service agreement

A Service Agreement and Schedule of Supports are prepared once a direction is agreed.

    Prepared promptly

  7. 7

    Transition

Rosters, handovers and any clinical plans are set up before the first shift.

    Sorted before day one

  8. 8

    Commencement

Supports begin, with a named contact for the participant, family and referrer.
