Urgent & Last-Minute Referrals — Compassionate Community Care
All Services Compassionate Community Care Service

Urgent and last-minute NDIS referrals.

When a participant needs support to start at short notice, the useful thing is a fast, honest answer. Urgent referrals come straight to management and are screened immediately.

Screening

Immediate, with escalation directly to management.

Situations

Homelessness risk, delayed discharge, tenancy breakdown, custody release.

Capacity

Properties and an established workforce maintained for exactly this.

Limits

Assessment and compatibility work is never skipped.

Service overview

Fast is a capacity problem, not a paperwork problem.

Most providers are slow to urgent referrals for the same reason: they do not have a property or the staff, and it takes them two weeks to establish that. We maintain access to suitable properties and an established workforce specifically so we can answer the question quickly — including when the answer is no.

What we will not do is shorten the assessment. For a SIL placement we still complete compatibility and risk assessment, because a placement that breaks down in a month leaves the participant worse off than the delay would have.

Who This Service Is For

Support Coordinators, hospital discharge teams, guardians, justice and community agencies, and families dealing with a situation that cannot wait.
