Service overview

A transition is a project, not a start date.

Delayed discharges and failed releases usually have the same cause: everybody focused on the date and nobody owned the fortnight around it. Somebody needs to have the house ready, the roster built, the medications sorted, the appointments booked and the stakeholders talking to each other before the participant walks out the door.

That is the work we take on. We do the planning with the hospital or the justice and community agencies, get the accommodation and staffing in place, and then support intensively in the early weeks before stepping back as things settle.
